Application Forms
For your convenience, the following application forms are available for downloading in the Adobe Acrobat® (pdf) format. If you need the Acrobat Reader software, you can download it for free from Adobe's website by clicking on the logo below.
Five electronic copies and, one unbound original copy of a complete application must be received prior to the NOON on the 10th calendar day of the month. If the 10th calendar day falls on a weekend or state holiday, all applications are due by Noon the preceding business day. The first deadline that this resolution applies to is September 10, 2009.
Beginning October 20th, 2009, only applications submitted in the above format will be accepted.
ALL CORRESPONDENCE either to or copied to IJDC has the IJDC project number listed on the cover. If an IJDC project number is pending (for newly submitted applications, etc.) then the cover should indicate that the IJDC project number is pending. If correspondence is received on or after September 1, 2009 without the IJDC Project number or an indication that the project number is pending, the correspondence will be returned with a standard letter referencing the policy.
